With the 2026 midterm elections drawing closer, one piece of legislation comes up in nearly every conversation about the direction of the country: the One Big Beautiful Bill Act. Signed into law on July 4, 2025, the law is the central piece of President Donald Trump‘s second-term domestic agenda. Its official designation is H.R. 1, Public Law 119-21.

The bill passed through the budget reconciliation process — a procedural path that allows certain fiscal legislation to pass the Senate with a simple majority rather than the standard 60-vote threshold. The Senate passed it 51–50 on July 1, 2025, with Vice President JD Vance casting the tiebreaking vote. The House of Representatives passed the Senate version 218–214 on July 3, 2025. President Trump signed it into law the following day.

The law contains hundreds of provisions spanning taxes, health programs, food assistance, student loans, border security, national defense, and energy policy. What follows is a plain-English guide to what each major section of the law actually says.

Taxes

The tax title is the largest component of the One Big Beautiful Bill Act and the one with the most immediate impact on American households.

The law permanently extends the individual income tax rates established by the 2017 Tax Cuts and Jobs Act (TCJA). Those rates were scheduled to expire at the end of 2025, which would have triggered automatic increases for most taxpayers in 2026. The OBBBA cancels that expiration and makes the TCJA rates permanent.

The law also creates several new temporary tax provisions, all retroactive to January 1, 2025. Workers who receive tips may deduct up to $25,000 in qualified tip income if filing jointly, or up to $12,500 if filing as a single filer. This provision expires at the end of 2028. Workers who receive overtime pay may deduct up to $12,500 in qualified overtime compensation annually. This provision also expires at the end of 2028. Taxpayers 65 years of age and older receive a new $6,000 deduction, also expiring in 2028. Taxpayers who take out auto loans may deduct up to $10,000 in interest annually, expiring in 2028.

The standard deduction is increased and made permanent. The Child Tax Credit is raised to $2,200 per child for tax years 2025 through 2028 and made permanent at $2,000 per child beginning in 2026. 100 percent bonus depreciation for qualifying business investment is restored and made permanent. The estate tax exemption is permanently increased.

The State and Local Tax (SALT) deduction cap, which has been set at $10,000 since 2017, is raised to $40,000 for taxpayers with incomes below $500,000. This increase is effective for tax years 2025 through 2029, after which the cap reverts to $10,000.

The law creates Trump Accounts — tax-deferred savings and investment accounts for children. The federal government provides a $1,000 seed deposit at birth for eligible children. Individual contributions are capped at $5,000 per year per child. Employer contributions are capped at $2,500 per year. Funds must be invested in qualifying mutual funds or exchange-traded funds that track a U.S. stock index such as the S&P 500.

The law also establishes the first national school choice program at the federal level. Beginning January 1, 2027, taxpayers may claim a dollar-for-dollar nonrefundable federal tax credit of up to $1,700 per year for cash contributions to qualifying Scholarship Granting Organizations (SGOs). These organizations in turn award scholarships to eligible K–12 students for qualifying education expenses including tuition, fees, books, supplies, tutoring, uniforms, and technology. Eligible students must be from households earning below 300 percent of their area’s median income. States must elect to participate in the program and submit a list of qualifying SGOs to the IRS. The program takes effect for tax year 2027.

Medicaid

The OBBBA introduces the most significant changes to Medicaid since the program’s expansion under the Affordable Care Act (ACA). Federal Medicaid spending reductions under the law total $886.8 billion over the 2025–2034 period, per CBO‘s October 2025 supplemental analysis.

The law imposes an 80-hour-per-month community engagement requirement on able-bodied adults ages 19 to 64 who are enrolled in Medicaid through ACA expansion. Enrollees must document that they are working, in job training, enrolled in an educational program, or performing community service for at least 80 hours per month to maintain eligibility. Exemptions apply to parents of children 13 years old and under, pregnant and postpartum individuals, and those certified as medically frail. States must implement this requirement no later than December 31, 2026.

The law also reduces the frequency of eligibility redeterminations from every 12 months to every 6 months, meaning enrollees must re-verify their eligibility twice as often as before. This provision takes effect upon enactment.

The law restricts states’ ability to use provider taxes — fees charged to hospitals and health systems — to generate the state share of Medicaid funding. This mechanism has been widely used by states across the country to maximize federal matching funds.

A one-year ban on Medicaid funding to abortion providers is included in the law.

To offset the impact of these changes on rural health infrastructure, the law establishes the Rural Health Transformation Program, which appropriates $10 billion per year from 2026 through 2030 — $50 billion total — for states to support rural hospitals and health facilities. States were required to submit applications to the Centers for Medicare and Medicaid Services (CMS) by December 31, 2025. States that newly adopt Medicaid expansion after January 1, 2026 will no longer receive the enhanced federal matching rate that was previously available to new expansion states.

SNAP

The OBBBA makes the largest changes to the Supplemental Nutrition Assistance Program (SNAP) — formerly known as food stamps — in the program’s history. The law reduces federal SNAP spending by $186 billion over the 2025–2034 period, representing approximately 20 percent of projected program spending. More than 42 million Americans currently receive SNAP benefits.

The law expands SNAP work requirements to able-bodied adults ages 18 through 64. Under prior law, the requirement applied to adults ages 18 through 54. The new requirement — which took effect November 1, 2025 — mandates that covered individuals work, participate in job training, or perform volunteer service for at least 80 hours per month to maintain eligibility. Adults who fail to meet the requirement may receive benefits for no more than three months in any 36-month period.

Several groups previously exempt from the requirement are now covered under the new rules. Parents and caretakers of children 14 years of age and older are now subject to work requirements. Veterans, homeless individuals, and former foster youth no longer receive automatic exemptions.

The law redefines the Thrifty Food Plan — the federal formula used to calculate SNAP benefit levels — in a way that limits future increases to benefit amounts. This change takes effect upon enactment.

Beginning in 2027, the administrative cost-sharing arrangement between the federal government and states shifts from a 50–50 split to a 75 percent federal and 25 percent state contribution. Beginning in 2028, states whose SNAP payment error rates exceed 6 percent will be required to share in program benefit costs as well.

The law also removes SNAP eligibility for certain categories of legal non-citizens, including refugees, asylees, and recipients of certain forms of humanitarian parole who were previously eligible for the program.

Student Loans

The OBBBA restructures federal student lending beginning July 1, 2026. The 2025–26 academic year is not affected. All changes apply to new loans disbursed on or after July 1, 2026.

The Grad PLUS loan program — which previously allowed graduate and professional students to borrow up to the full cost of attendance — is eliminated for new borrowers beginning July 1, 2026. Students already enrolled in a credentialed program who borrowed a federal loan before that date retain access to Grad PLUS for up to three additional academic years or until program completion, whichever comes first.

New annual and lifetime borrowing caps take effect on July 1, 2026. Graduate students are capped at $20,500 per year in unsubsidized loans, with a $100,000 aggregate lifetime limit. Students in professional degree programs — including law and medicine — are capped at $50,000 per year, with a $200,000 aggregate lifetime limit. Parent PLUS loans are capped at $20,000 per student per year, with a $65,000 lifetime limit per dependent student.

The income-driven repayment landscape is restructured for new borrowers. The SAVE Plan, Pay As You Earn (PAYE), and Income-Contingent Repayment (ICR) are eliminated for loans disbursed on or after July 1, 2026. Borrowers with existing loans in these plans must transition to a new plan by June 30, 2028. Income-Based Repayment (IBR) remains available for borrowers who do not take out new loans after July 1, 2026. The SAVE Plan had already been blocked by federal courts prior to the OBBBA‘s enactment.

New borrowers will have two repayment options. The first is a tiered Standard Repayment Plan with fixed payments over a term of 10 to 25 years depending on total loan balance. The second is the new Repayment Assistance Plan (RAP), which calculates monthly payments at 1 to 10 percent of adjusted gross income, with a $10 monthly minimum. Each dependent claimed on a borrower’s tax return reduces the monthly payment by $50. Any remaining balance is forgiven after 360 qualifying monthly payments — 30 years.

Border Security and Defense

The OBBBA represents the largest single investment in border security and immigration enforcement since the creation of the Department of Homeland Security in 2003. Total new funding for border and interior enforcement activities across all relevant agencies totals more than $170.7 billion, available through September 30, 2029.

The law appropriates $46.55 billion to Customs and Border Protection (CBP) for border wall construction, installation, and improvement of physical barriers along the southern and northern borders, along with access roads, cameras, lights, sensors, and related technology. The White House has stated the funding supports construction of 701 miles of primary wall, 900 miles of river barriers, 629 miles of secondary barriers, and 141 miles of additional infrastructure.

The law appropriates $45 billion to Immigration and Customs Enforcement (ICE) for adult and family residential detention facilities, available through FY2029. $29.9 billion is appropriated to ICE for enforcement and deportation operations, including hiring and training of additional agents, transportation and removal operations, fleet modernization, and expansion of 287(g) agreements with state and local law enforcement. The total ICE funding increase through 2029 brings the agency’s budget from approximately $10 billion annually to more than $100 billion over the funding window, making it the single most funded federal law enforcement agency in the country.

$4.1 billion is appropriated for hiring and training new Border Patrol agents and CBP support personnel. An additional $2 billion is provided for retention, hiring, and performance bonuses. $6.2 billion funds border security technology and surveillance systems. $5 billion is allocated for CBP facility construction and improvement.

A $10 billion State Border Security Reinforcement Fund is established to reimburse states for border barrier construction, interdiction activities, and relocation of illegal aliens. Notably, this fund covers qualifying activities dating back to January 2021. A separate $3.5 billion BIDEN Reimbursement Fund — formally the Bridging Immigration-related Deficits Experienced Nationwide fund — reimburses states and local jurisdictions that have assisted in apprehending, prosecuting, transporting, and detaining criminal aliens.

New fees are imposed on immigration benefit applications. A $100 fee is required to apply for asylum, with an additional $100 annual fee for each year a case remains pending. A $550 fee applies to initial work authorization applications, with a $275 renewal fee.

The law appropriates $150 billion in new national defense spending, including funding for military personnel, equipment, and infrastructure.

Energy

The OBBBA eliminates or phases out a series of clean energy tax credits established under prior law while maintaining and expanding support for domestic fossil fuel production.

The $7,500 federal tax credit for the purchase of new electric vehicles under Section 30D was terminated effective September 30, 2025. The $4,000 credit for used electric vehicles under Section 25E was also terminated. The Energy Efficient Home Improvement Credit under Section 25C — which covered insulation, windows, doors, and heat pumps — was terminated for property placed in service after December 31, 2025. The Residential Clean Energy Credit under Section 25D — which covered solar panels, battery storage, fuel cells, and geothermal systems — was terminated for expenditures made after December 31, 2025.

Wind and solar electricity production credits remain available for projects that begin construction no later than July 5, 2026, or that are placed in service by the end of 2027.

The Greenhouse Gas Reduction Fund, established under the Inflation Reduction Act, is eliminated. Federal penalties for noncompliance with Corporate Average Fuel Economy (CAFE) standards are eliminated. The law includes provisions expanding oil and gas leasing access on federal lands and streamlining permitting for domestic energy production projects.

The One Big Beautiful Bill Act — H.R. 1, P.L. 119-21 — was signed into law on July 4, 2025. It is the product of the budget reconciliation process under the 119th United States Congress. Its provisions span taxes, health programs, food assistance, student loans, border security, national defense, and energy policy. President Donald Trump delivered his 2026 State of the Union address to a joint session of Congress on February 24, 2026, speaking for one hour and 47 minutes in what the White House described as the longest such address in modern history. Trump declared the state of the union strong, framing his administration’s first year as a historic economic turnaround and presenting a series of specific, numerical claims about investment, inflation, employment, energy, and health care.

Investment and Markets

Trump’s claim: “In 12 months, I secured commitments for more than $18 trillion pouring in from all over the globe.” He also stated that the Dow Jones Industrial Average broke 50,000 four years ahead of schedule, that the S&P 500 hit 7,000, and that the stock market set 53 all-time record highs since the election.

The stock market claims were not disputed by major fact-checkers. The Dow did exceed 50,000 during 2025, and the S&P 500 did reach 7,000. The 53 record highs figure was reported as stated without challenge.

The $18 trillion investment claim drew immediate scrutiny. The White House Investment Tracker, the administration’s own public-facing record of investment commitments, listed a total of approximately $9.6 to $9.7 trillion as of its most recent update — roughly half the figure Trump stated on the floor of Congress. A White House official confirmed to ABC News that the $9.7 trillion figure covers “investments that have materialized or committed” but did not explain the gap between that number and the president’s $18 trillion claim. An analysis by Bloomberg Economics found that more than $2.5 trillion of the White House’s own $9.7 trillion figure does not represent actual investment but rather vague commitments to increase trade or bilateral economic relationships. An additional $3.5 trillion consists of opaque sovereign pledges, and more than $250 billion of the total was announced or planned before Trump took office in January 2025. Fact-checks published February 25, 2026 by NBC News, CNN, CBS News, and NPR all concluded the $18 trillion figure could not be verified through public records.

Inflation and Cost of Living

Trump’s claim: His administration had driven core inflation down to its lowest level in more than five years, with core inflation running at 1.7% in the final three months of 2025. He also stated that the price of eggs is down 60%, that the annual cost of a typical new mortgage is down almost $5,000 since he took office, and that the costs of chicken, butter, hotels, automobiles, and rent are all lower than when he was inaugurated.

The Bureau of Labor Statistics released its January 2026 Consumer Price Index report on February 11, 2026. The headline CPI rose 2.4% on a 12-month basis as of January 2026, down from 2.7% for the year ending December 2025 and 3.0% when Trump took office in January 2025. Core CPI, which excludes food and energy, came in at 2.5% year-over-year as of January 2026 — the lowest annual core reading since March 2021. The monthly core rate averaged 0.2% per month in the final quarter of 2025, which translates to an annualized rate of approximately 2.4% to 2.9%, not the 1.7% Trump cited. Headline and core inflation have fallen since Trump took office, but no standard BLS measure produced a 1.7% reading in the fourth quarter of 2025.

On eggs, the BLS data show the egg price index fell approximately 48% from January 2025 to January 2026, and 34% year-over-year as of the January 2026 report, according to CNBC. Trump’s stated figure of 60% is higher than the BLS-measured decline. On the broader food picture, BLS data for January 2026 show overall food prices were up 2.9% year-over-year. Prices for food at home rose 2.1% and nonalcoholic beverages — including coffee — rose 4.5% over the same period.

On mortgages, Freddie Mac’s Primary Mortgage Market Survey as of February 26, 2026 — two days after the SOTU — showed the 30-year fixed-rate mortgage averaging 5.98%. One year earlier, the same survey showed the rate at 6.76%. That is a decline of 78 basis points over 12 months and marks the first time since September 2022 that the 30-year rate averaged below 6%. The direction of Trump’s claim — that mortgage costs have fallen since his inauguration — is supported by Freddie Mac data.

Jobs and Employment

Trump’s claim: More Americans are working today than at any time in the history of the country. 100% of all jobs created under his administration have been in the private sector. 2.4 million Americans have been lifted off food stamps — a record. The administration has added 70,000 new construction jobs.

The BLS Employment Situation report for January 2026, released February 11, 2026, showed approximately 158.5 million Americans employed — a record high. The total number of employed Americans has risen since Trump took office in January 2025, when approximately 157 million people were employed. However, economists note that total employment typically rises with population growth. CBS News reported that the labor force participation rate in January 2026 stood at 62.5% — identical to the rate in December 2024, Biden’s last full month in office — and the unemployment rate ticked up under Trump to 4.3% from 4.1% in December 2024.

On private sector job creation, federal government employment fell by 34,000 in January 2026 alone, as employees who accepted deferred resignation offers in 2025 came off federal payrolls, according to BLS. Private sector gains were real in the same period.

On food stamps, NBC News reported that the 2.4 million figure refers to the number of Americans the Center on Budget and Policy Priorities, a nonpartisan think tank, projects will lose eligibility for the Supplemental Nutrition Assistance Program as a result of new work requirements included in the One Big Beautiful Bill signed in July 2025 — not Americans who left the program due to improved economic circumstances. Approximately 42 million Americans remain enrolled in SNAP as of the most recent available data.

On construction jobs, BLS data show 44,000 construction jobs were added from January 2025 to January 2026 — roughly 26,000 fewer than the 70,000 Trump cited.

Energy

Trump’s claim: American oil production is up by more than 600,000 barrels per day. American natural gas production is at an all-time high. The United States has received more than 80 million barrels of oil from Venezuela. Gas prices are below $2.30 in most states.

On oil production, the U.S. Energy Information Administration’s February 2026 Short-Term Energy Outlook shows U.S. crude oil production averaged 13.60 million barrels per day in 2025, compared to 13.235 million barrels per day in 2024 — an annual average increase of approximately 365,000 barrels per day. EIA data show the single highest monthly production figure on record was October 2025 at 13.864 million barrels per day. The 600,000 barrels per day figure depends on the specific baseline and comparison period used, and exceeds the annual average increase reported by EIA.

On natural gas, the EIA’s February 2026 STEO forecasts record U.S. dry natural gas production in both 2026 and 2027, and production rose throughout 2025. NPR’s SOTU fact-check found the natural gas production claim to be accurate.

On Venezuela, Trump’s statement that the United States has received more than 80 million barrels of oil from Venezuela followed the January 3, 2026 U.S. military operation in Caracas that led to the capture of Venezuelan President Nicolás Maduro, who was extradited to New York on narcoterrorism charges. The 80 million barrel figure was not disputed by major fact-checkers. Secretary of State Marco Rubio had previously stated in January that the U.S. planned to take between 30 and 50 million barrels of Venezuelan oil already produced and packaged and sell it in the marketplace. Some experts quoted by Al Jazeera noted that the logistics of oil transfers are complex and that questions remain about whether all reported volumes represent commercial flows. The administration’s characterization of Venezuela as a “new friend and partner” reflects the change in Caracas’s government following Maduro’s removal.

On gasoline prices, GasBuddy, which tracks approximately 150,000 stations nationwide, reported that the cheapest 10% of all U.S. stations had gas priced at $2.30 as of February 23, 2026 — the day before the SOTU. The national average as reported by AAA was approximately $2.92 per gallon as of the same period. Only eight stations nationwide were selling a gallon of gas for under $2.00, according to CBS News. The $2.30 figure represents the price floor at the least expensive stations, not a national average or a majority-of-states figure.

Health Care and Prescription Drugs

Trump’s claim: His administration has taken prescription drug prices from the highest in the world to the lowest. Differences of 300% to 600% are now visible and available at trumprx.gov. He also called on Congress to pass the Great Health Care Plan to stop payments to big insurance companies and send money directly to the American people.

TrumpRx.gov launched as a functional direct-to-consumer portal on February 5, 2026 — 19 days before the SOTU. The site lists 43 brand-name medications from five pharmaceutical manufacturers — AstraZeneca, Eli Lilly, EMD Serono, Novo Nordisk, and Pfizer — that have reached pricing agreements with the administration. Discounts on listed drugs range from 33% to 93% measured against the drugs’ list prices. Examples include Eli Lilly’s Zepbound starting at $299 per month, down from a list price of approximately $1,088, and Novo Nordisk’s Wegovy pill at as little as $149 per month, down from a list price of approximately $1,349. The administration said more manufacturers and drugs will be added in coming months.

Health policy experts cited by CNN, NPR, and STAT News at launch noted that the discounts are calculated against list prices, which often do not reflect what insured patients actually pay after rebates and negotiations. The site requires cash payment and cannot be used in conjunction with Medicare or Medicaid. STAT News found that approximately 18 of the 43 listed brand-name drugs are available as generics at lower prices through other platforms. The site itself cautions users that their insurance copay may be lower than the TrumpRx cash price.

Trump’s claim that no prior president had lowered drug prices was disputed. NBC News noted that President Joe Biden signed the Inflation Reduction Act in 2022, which capped insulin at $35 per month for Medicare beneficiaries, imposed a $2,000 annual out-of-pocket cap on drug costs for Medicare enrollees, and authorized Medicare to negotiate prescription drug prices directly with manufacturers for the first time.

The Great Health Care Plan is a legislative framework Trump introduced on January 15, 2026, calling on Congress to enact it into law. It is not a signed law or executive order. Key provisions include codifying the administration’s Most-Favored-Nation drug pricing deals, reinstating ACA cost-sharing reduction payments, sending premium subsidy money directly to eligible Americans rather than to insurers, requiring insurance companies to publish claims denial rates and overhead costs in plain English, and mandating price transparency for any provider or insurer accepting Medicare or Medicaid. The Committee for a Responsible Federal Budget estimated the cost-reducing provisions could reduce primary deficits by approximately $50 billion over a decade, though the ACA subsidy provision could add as much as $350 billion in costs depending on its final design. As of the February 24 SOTU, Congress had not acted on the proposal.

On December 2, 2025, President Donald Trump convened his ninth and final cabinet meeting of the year in the White House Cabinet Room. Twenty cabinet secretaries and Vice President JD Vance delivered updates on their departments’ achievements and priorities. The session ran two hours and 18 minutes and was open to the press. Every statement made that day is part of the official record.

Eight weeks later, on February 24, 2026, Trump delivered his State of the Union address to a joint session of Congress, updating many of those same themes before the largest national audience of his second term.

Treasury: The One Big Beautiful Bill

At the December 2 meeting, Treasury Secretary Scott Bessent described a package of pending tax legislation he said would transform the U.S. economy. The bill would deliver 100% business expensing, eliminate taxes on tips and overtime pay, remove taxes on Social Security benefits for seniors, and allow retroactive car loan deductions. Bessent described the bond market as having its best year since 2020 and said the legislation, if passed, would set up a strong 2026 economy.

At the February 24 SOTU, Trump confirmed the bill had passed, telling Congress it delivered no tax on tips, no tax on overtime, no tax on Social Security for seniors, and made interest on auto loans tax deductible for American-made vehicles. Trump credited Republican majorities for passing the bill and noted every House and Senate Democrat voted against it.

What the record shows: According to AAA, the national average for a gallon of regular gasoline as of February 19, 2026 was $2.92, down from $3.14 at the same point one year earlier. Gas prices declined year-over-year in 48 of 50 states. The ten least expensive markets as of mid-February were Oklahoma ($2.28), Arkansas ($2.45), Kansas ($2.46), Mississippi ($2.49), Iowa ($2.50), Louisiana ($2.50), Missouri ($2.51), North Dakota ($2.53), Wisconsin ($2.53), and Texas ($2.55). The ten most expensive markets were led by California ($4.59) and Hawaii ($4.40). The U.S. Energy Information Administration forecasts the national average for all of 2026 at $2.91 per gallon. The national average of $2.92 as of February 19 remains above the $2.30 figure Trump cited at the SOTU.

Homeland Security: Immigration Enforcement

At the December 2 meeting, Homeland Security Secretary Kristi Noem reported the removal of 2 million illegal immigrants since January 20, 2025, with the administration prioritizing the removal of terrorists and criminals. Noem also announced the hiring of the 10,000th ICE officer under the administration.

At the February 24 SOTU, Trump said the border is now the strongest and most secure in American history, that zero illegal aliens have been admitted to the United States in the past nine months, and that deadly fentanyl crossing the border is down by a record 56% in one year.

What the record shows: The Department of Homeland Security, in a statement dated February 24, 2026, reported that nearly 3 million individuals have left the United States since January 20, 2025, including an estimated 2.2 million self-deportations and more than 713,000 deportations. The DHS figures combine voluntary departures tracked through the CBP Home app with formal ICE removals.

Independent researchers have disputed the self-deportation methodology. The Brookings Institution estimates actual ICE removals for 2025 at 310,000 to 315,000. The Deportation Data Project puts the figure at approximately 350,000 deportations through early February 2026. Both organizations have published detailed explanations of their methodologies. DHS disputes the lower figures.

The core disagreement centers on how self-deportations are counted. DHS uses Current Population Survey data and CBP app registrations to estimate voluntary departures. Brookings and other independent researchers have said those data sources cannot reliably measure the number of people who actually left the country.

Defense: Drug Boat Strikes and Narco Interdiction

At the December 2 meeting, Defense Secretary Pete Hegseth reported a 91% reduction in sea-based drug entries into the United States, attributing it to ongoing lethal military strikes against vessels the administration designated as narco-terrorist targets. Hegseth described the designation of Latin American cartels as foreign terrorist organizations, enabling military authority to treat their boats as legitimate strike targets. Trump interjected at the meeting, stating each boat taken out saves an estimated 25,000 American lives.

At the February 24 SOTU, Trump cited fentanyl crossing the border as down 56% and described the administration as engaged in armed conflict with drug cartels in Latin America.

What the record shows: Since September 2025, U.S. Southern Command has conducted at least 38 to 40 strikes on vessels in the Caribbean Sea and Eastern Pacific Ocean under what has been identified as Operation Southern Spear. As of mid-February 2026, at least 135 people have been killed in the strikes, according to reporting by Stars and Stripes, NBC News, CNN, and USNI News. No U.S. military personnel have been reported killed or injured in the operations.

The operation produced a major geopolitical development on January 3, 2026, when U.S. military forces conducted an operation in Caracas, Venezuela, resulting in the capture of Venezuelan President Nicolás Maduro and his wife, Cilia Flores. Both were extradited to New York to face federal charges including narcoterrorism conspiracy and cocaine importation conspiracy. Maduro has pleaded not guilty to all charges. Acting Venezuelan President Delcy Rodríguez has maintained that Maduro remains the country’s legitimate leader.

Strike frequency slowed following Operation Absolute Resolve and Maduro’s capture. On February 5, 2026, Hegseth posted on social media that “some top cartel drug-traffickers” had “decided to cease all narcotics operations indefinitely” due to the strikes. Hegseth did not name the cartels and provided no supporting data. Strikes have continued, with three vessels struck on February 16 killing 11 people, and additional strikes in the weeks since.

The operations have drawn scrutiny from Congress and international bodies. The Senate and House Armed Services Committees have opened bipartisan investigations. The United Nations Human Rights Chief called for an investigation, citing what he described as “strong indications of extrajudicial killings.” Congressional lawmakers have not moved to halt the strikes. The original 91% sea drug reduction figure cited in December has not been updated with a new independently verified number.

Health and Human Services: Prior Authorization Reform

At the December 2 meeting, HHS Secretary Robert F. Kennedy Jr. stated the department had eliminated prior authorizations for 280 million Americans by January 2026 and had enabled health record sharing that increased patient access 100-fold.

What the record shows: In June 2025, HHS secured a voluntary, non-binding pledge from insurance industry representatives to reduce the volume of medical services subject to prior authorization by January 1, 2026. The pledge also called for real-time approvals for most requests by 2027 and required that medical professionals review all clinical denials. The commitment was announced jointly by Secretary Kennedy and CMS Administrator Dr. Mehmet Oz at an HHS event.

The pledge was a voluntary industry commitment, not a federal regulation, mandate, or enforceable legal requirement. No HHS announcement confirming the completed elimination of prior authorizations for 280 million Americans has been issued. At a Heritage Foundation event on February 9, 2026, Kennedy said the administration “will continue to reform prior authorization” — language indicating the work remains in progress.

Kennedy’s broader HHS agenda has included a significant restructuring of the department, reducing its workforce from approximately 82,000 to 62,000 employees. That reorganization was partially blocked by a federal court in Rhode Island, which ruled in New York v. Kennedy that 19 states and the District of Columbia were likely to succeed in their claim that the reduction-in-force was conducted unlawfully. The court order prevented further reorganization steps but did not reinstate terminated employees. Kennedy’s most-favored-nation drug pricing policy was enacted and is accessible to consumers at trumprx.gov.

What Comes Next
